Summary:
Whereas, the purpose of this Memorandum of Understanding (this MOU) is a formalized partnership between Advanced Disability Management Services (“ADMS” or Partner) and the Newark Board of Education as the partner will provide support coordination services to district high school child study team members during the 2024-2025 school year.

Whereas, the presentations will be provided in the areas of DDD and Medicaid Applications Overview of the NJ Division of Developmental Disabilities (DDD) application process, explanation of the Medicaid application process, (which is a DDD requirement), discussion of the DDD Tiers/budgets levels that are assigned upon enrollment in DDD, overview of the NJ Comprehensive Tool (NJ CAT), overview of services available through clients DDD budgets, importance of having a Support Coordinator, residential placement and the criteria for accessing this service.

Now, Therefore, Be Resolved, the Newark Board of Education approves the Memorandum of Understanding between the Newark Board of Education and Advanced Disability Management Services.
